Received recognition as a Level II Patient Centered Medical Home (PCMH) from the National Center for Quality Improvement. PCMH medical practices provide a system of care where “patients get the care they want and need, when they want and need it,” by their own care teams. We are very proud of this achievement and we look forward to maintaining this status going forward.
